Dean's Certification and Transfer Forms
Students who need verification of their conduct status for transfer applications or other institutional requests can submit their Dean’s Certification or Transfer Forms pertaining to Student Conduct or disciplinary history using the guidelines below.
BEFORE YOU SUBMIT YOUR FORM
Please review the following prerequisites to ensure your request can be processed smoothly:

Complete your portion of the form with your information.

Please use your CCP email whenever possible to submit requests.

Sign and date your form, including the waiver of right to access (You have the right to not waive viewing privileges and request a copy, but be aware that this will invalidate the form, making it unofficial).

Without a signature, we cannot process your form. Please apply it as a stamp or insert it manually next to your name, should there not be a dedicated space.

Include your J# so your records can be verified.

If not indicated on the form, please include the email address of your transfer institution’s admissions office or an appropriate institutional staff member. We will send the form directly to them.

Once this is complete, we will notify you of the transmission. Your chosen transfer institution will confirm receipt with you directly, or you may contact their admissions office to ensure they are in receipt of the document.

HOW TO SUBMIT YOUR REQUEST

Review the prerequisites above.

Attach your completed form to your email.
- Email your request to transferforms@ccp.edu.

The Office of the Dean of Students submits the completed form directly to your new institution. You will not receive a copy.

The processing time is at least one week. Please be aware of your transfer institution deadlines and plan accordingly.

If you have questions about your form or the process, please email transferforms@ccp.edu. Please do not contact the Dean of Students directly.
IF YOUR TRANSFERRING INSTITUTION DOES NOT HAVE A FORM

The Office of the Dean of Students does not draft individualized letters on College letterhead.

Instead, the office completes official transfer or Dean’s Certification forms that confirm whether a student has any conduct history.
- If no form is provided, students should ask the requesting institution to email an official request to transferforms@ccp.edu.

The office responds directly to the requesting institution. You will not receive a copy.
ADDITIONAL NOTES

This email account is used only for student conduct and disciplinary form processing. When The Office of the Dean of Students receives the request, we check your records, the form receives a signature, and then we remit this form directly to the Institution. You will not receive an attached copy. However, we can confirm via email or phone call that the form was sent.

Please be mindful when the college is officially closed or on break. Staff, Administrators, nor Faculty will review emails received during this time.

Academic transcripts and all academic record inquiries are handled by the Office of Records and Registration.
