You need to provide the name and and address of at least one physician and a prescription from that physician to provide the technician on April 16 at your appointment. You need to bring your last mammography film for comparison purposes unless it was performed by Fox Chase Cancer Center. I have request forms for a previous mammogram if needed.

You CANNOT gat a mammogram on the van if you were diagnosed with breast cancer within the last 3 years, currently being treated for breast cancer, have a new breast problem (feeling a lump, skin changes, nipple discharge), if you are breast feeding or have breast fed in the last 9 months or may be pregnant.

You can get a mammogram on the van if you are age 40 and older once a year. You may get one baseline mammogram if you are 35 to 39 years of age.

There are no costs or co-payments for a mammogram.
